OBJECTIVES: To determine the effect of neighborhood ethnic composition on medical scooters prescriptions. DESIGN: The 5% noncancer sample of Medicare recipients in the Surveillance, Epidemiology and End Results (SEER)-Medicare linked database, from 1994 to 2001. SETTING: SEER regions. PARTICIPANTS: Individuals covered by Medicare living in SEER regions without a cancer diagnosis. MEASUREMENTS: Individual characteristics (age, sex, ethnicity, justifying diagnosis, and comorbidity), primary diagnoses, neighborhood characteristics (percentage black, percentage Hispanic, percentage with <12 years education, and median income), and SEER region. RESULTS: The rate of power wheelchair prescriptions was 33 times greater in 2001 than in 1994, with a shift over time from justifying diagnoses more closely tied to mobility impairment, such as strokes, to less-specific medical diagnoses, such as osteoarthritis. In multilevel, multivariate analyses, individuals living in neighborhoods with higher percentages of blacks or Hispanics were more likely to receive power wheelchairs (odds ratios=1.09 for each 10% increase in black residents and 1.23 for each 10% increase in Hispanic residents) after controlling for ethnicity and other characteristics at the individual level. CONCLUSION: These results support allegations that marketers promoting power wheelchairs and handicap scooters have specifically targeted minority neighborhoods

Fighting Internet Spam

How prevalent is Spam? According to Scott McAdams, OMA Public Affairs and Communications Department (oma.org):
“Studies show unsolicited or “junk” e-mail, known as spam, accounts for roughly half of all e-mail messages received. Although once regarded as little more than a nuisance, the prevalence of spam has increased to the point where many users have begun to express a general lack of confidence in the effectiveness of e-mail transmissions, and increased concern over the spread of computer viruses via unsolicited messages”.
In 2003, President Bush signed the “Can Spam” bill, in December of 2003 which is the first national standards around bulk unsolicited commercial e-mail. The bill, approved by the Senateby a vote of 97 to 0, prohibits senders of unsolicited commercial e-mail from using false return addresses to disguise their identity (spoofing) and the use of dictionaries to generate such mailers. In addition, it prohibits the use of misleading subject lines and requires that emails include and opt-out mechanism. The legislation also prohibits senders from harvesting addresses off Web sites. Violations constitute a misdemeanor crime subject to up to one year in jail.
One major point that needs to be discussed about this: spam is now coming from other countries in ever-greater numbers. These emails are harder to fight, because they come from outside our countrys laws and regulations. Because the Internet opens borders and thinks globally, these laws are fine and good, but do not stop the problem.
So what do you do about this? Her are the top 5 Rules to do to protect from spam.
Number 1: Do what you can to avoid having your email address out on the net. There are products called “spam spiders” that search the Internet for email addresses to send email to. If you are interested, do a search on “spam spider” and you will be amazed at what you get back. Interestingly, there is a site, WebPoison.org, which is an open source project geared to fight Internet “spambots” and “spam spiders”, by giving them bogus HTML web pages, which contain bogus email addresses
A couple suggestions for you:
a) use form emails, which can hide addresses or also
b) use addresses like sales (at) company.com instead of your full address to help battle the problem.
c) There are also programs that encode your email, like jsGuard, which encodes your email address on web pages so that while spam spiders find it difficult or impossible to read your email address.
Number 2: Get spam blocking software. There are many programs out there for this. (go to cloudmark.com or mailwasher.net for example). You may also buy a professional version. Whatever you do, get the software. It will save you time. The software is not foolproof, but they really do help. You usually have to do some manual set up to block certain types of email.
Number 3: Use the multiple email address approach. There are a lot of free email addresses to be had. If you must subscribe to newsletters, then have a “back-up” email address. It would be like giving your sell phone number to your best friends and the business number to everyone else.
Number 4: Attachments from people you don’t know are BAD, BAD, BAD. A common problem with spam is that they have attachments and attachments can have viruses. Corporations often have filters that don’t let such things pass to you. Personal email is far more “open country” for spamers. General rule of thumb: if you do not know who is sending you something, DO NOT OPEN THE ATTACHMENT. Secondly, look for services that offer filtering. Firewall vendors offer this type of service as well.
Number 5: Email services now have “bulk-mail” baskets. If what you use currently does not support this, think about moving to a new vender. The concept is simple. If you know someone, they can send you emails. If you don’t know them, put them in the bulk email pile and then “choose” to allow them into your circle. Spam Blocking software has this concept as well, but having extra layers seems critical these days, so it is worth looking into.

OBJECTIVE: To investigate whether the provision of power mobility would have an effect on body weight in adults who were first-time qualifiers for power mobility. DESIGN: This was a retrospective observational study of consecutive subjects, who served as their own controls. The medical scooters, handicap scooters records of 468 subjects who were approved for power mobility during a 17-mo period were reviewed. Three weight groups (12 mos before, at, and 12 mos after the power mobility evaluation) were evaluated with repeated-measures analysis of variance (ANOVA). The weight changes on subjects in different age groups (45-54, 55-64, 65-74, and >74), in geriatric vs. nongeriatric groups, and in different body mass index (BMI) groups were analyzed. RESULTS: Eighty-nine subjects met the inclusion criteria. They were obese (49.4%), and most of them were geriatric (64%). Congestive heart failure (30.34%) and chronic obstructive pulmonary disease (22.47%) were the two main presenting diagnoses. The repeated-measures ANOVA showed no significant weight change in the three studied weight groups. Similar results were seen in the age and BMI subgroups. CONCLUSIONS: There was no statistically significant weight change in adults who were first-time qualifiers and who used power mobility for 1 yr.

METHODS: Participants were randomly divided over an experimental group (n=14) and a control group (n=7). The experimental group received 7 weeks wheelchair training (3 week(-1), 70 min) at a low intensity (30% of the heart rate reserve), whereas the control group did not receive training. During pre- and post-tests, submaximal exercise was performed on a stationary wheelchair ergometer at fixed levels of power output. Mechanical efficiency, oxygen uptake, heart rate, timing parameters and stroke angles were measured. Video recordings were made to determine the stroke pattern. FINDINGS: Mechanical efficiency increased and metabolic cost decreased significantly in the experimental group compared to the control group. Push time increased and cycle frequency decreased as a result of training. The stroke angle increased in the experimental group during the training period. The experimental group preferred double-looping over propulsion, while the control group mainly used single-looping over propulsion patterns during the post-test. INTERPRETATION: A low-intensity, 7-week training protocol has a beneficial effect on the mechanical efficiency and metabolic cost of medical scooters propulsion in able-bodied participants. BACKGROUND: Almost 10% of all individuals who are legally blind also have a mobility impairment. The majority of these individuals are dependent on others for mobility. The Smart Power Assistance Module (SPAM) for manual wheelchairs is being developed to provide independent mobility for this population. METHODS: A prototype of the SPAM has been developed using Yamaha JWII power assist hubs, sonar and infrared rangefinders, and a microprocessor. The prototype limits the user to moving straight forward, straight backward, or turning in place, and increases the resistance of the wheels based on the proximity of obstacles. The result is haptic feedback to the user regarding the environment surrounding the medicare power wheelchairs. RESULTS: The prototype has been evaluated with four blindfolded able-bodied users and one individual who is blind but not mobility impaired. For all individuals, the prototype reduced the number of collisions on a simple navigation task. CONCLUSION: The prototype demonstrates the feasibility of providing navigation assistance to manual wheelchair users and medical scooters, but several shortcomings of the system were identified to be addressed in a second generation prototype.
